Required:

Locum medical officers confirmed dates of availability
Current signed and dated CV.
Copy of current AHPRA registration
Two references – no more than 12 months old and at least one from a current position
Certified copies of Drivers Licence and passport

It is mandatory for the locum to obtain a rebateable provider number prior to commencement.
